Omnivisor is the leading patient management hardware and software system that enables clincians to remotely manage patient care "by exception". This means that the Disease Management Software uses sophisticated algorithms to identify monitored patients with symptoms outside the parameters set by the health care provider or clinical guidelines. This process allows early intervention and helps to prevent unnecessary hospital admissions.

Omnivisor's powerful case and disease management program provides detailed analysis of patients' daily vital signs and symptoms data. This comprehensive system also includes patient demographics, healthcare and emergency contacts, medications, blood chemistries, and clinical notes. All monitoring parameters are fully programmable and the system can be integrated with existing data.

Commander

The Commander Home Monitor is an interactive home monitoring device which can assist in the management of Congestive Heart Failure (CHF), Diabetes, Chronic Obstructive Pulmonary Disease (COPD), Asthma and Coronary Artery Disease (CAD). The modular design allows you to select the Cardiocom peripheral devices that provide the most appropriate and cost-effective care for your patient. Patients can use their Commander Home Monitor to obtain objective biometric data such as: weight, temperature, blood pressure, blood glucose, peak flow, and oxygen saturation. In addition, patients are also required to answer a series of questions about their current symptoms. The data is then transmitted over the patient's telephone line directly to the Commander Data Management System for the clinician to access.

Commander Data Management System

The Commander Data Management System is located at the clinicians’ site and receives information daily about patients' vital signs and symptoms. Using intuitive software, the System automatically identifies "exception" patients (those with symptoms outside the parameters set by the patient's healthcare provider). This "management by exception" allows early intervention, including care plan adjustment, to improve patient care and reduce unnecessary stays in hospital. Since the database resides at your site, you have complete and immediate access to all of your patient information, including a broad base of patient and population reports for you to assess enrolment and outcomes.

Each day the patients can perform a health check using their telemonitoring Personal Link device in the comfort and privacy of their own home. The information is quickly and easily transmitted over the patient's phone line to the dietitian for analysis.
