AutoLink™ Blood Glucose Telemonitoring Device
AutoLink™ is a home telemonitoring device for the management of diabetes. In a matter of seconds, the cheque-book size GlucoCom monitor uses state-of-the-art telehealth technology to transmit data to the clinicians' management website. The easy to understand information can be quickly accessed by the individual for self-management and shared with the health care team so that medications, insulin or care plans can be adjusted to improved blood glucose control. Multiple readings are automatically transmiited over the telephone line to provide instant reporting of health status, compliance and test strip use.
Commander Multi-Biometric Telemonitoring Device
Commander is an interactive telehealth home device for those at high risk with chronic disease and in need of blood glucose, blood pressure, weight, oxygen saturation and peak flow monitoring. Thanks to its modular design, users can select the appropriate peripheral devices, such as the GlucoCom™ blood glucose meter. Commander features an extensive library of spoken and visual health check questions that can be customised for individual patients. Every day, compliance and biometric data can be transmitted over the telephone line directly to the managing clinician.
GlucoCom™ Trended Glucose Summary Reports and Diabetes Disease Management Software System
Daily blood glucose data transmitted from either the AutoLink™ or Commander monitoring devices can be used to automatically create a Glucose Summary Report for each patient. The Report provides trended data that can be accessed by the clinician. The Disease Management Software uses sophisticated algorithms to identify monitored patients with symptoms outside the parameters set by the health care provider or clinical guidelines. This efficient "management by exception" process allows early intervention to avoid unnecessary hospital admissions.
